Paulinder Rai, DO, MPH, CWS-P

Dr. Rai is a specialist in the field of Physical Medicine and Rehabilitation. His areas of expertise include sports medicine and non-surgical management of orthopedic injuries, pain management, post-stroke rehabilitation and spasticity, limb loss, and wound care.

Through his understanding of human anatomy, fitness, health, and nutrition he brings quality to the life of each of his patients. His comprehensive approach to health and disease helps him partner with his patients to create effective treatment plans and restore function. His philosophy is anchored in disease treatment and prevention, and he aims to facilitate the body’s natural tendency towards healing and restoration of balance using all medical modalities. Dr. Rai believes in evidence-based, common-sense medicine, and always keeps his patients at the center of his care.

Dr. Rai completed undergraduate training at New York University, where he studied philosophy, then earned his master’s degree in public health.  He earned his medical degree from The New York College of Osteopathic Medicine. He completed residency training in Physical Medicine and Rehabilitation at the State University of New York Downstate Medical Center. He is Board Certified in Physical Medicine & Rehabilitation.  He is a certified wound specialist physician (CWS-P) by the American Board of Wound Management. 

Dr. Tahsin Ashraf

Dr. Tahsin Ashraf is a specialist in Physical Medicine and Rehabilitation.  She is a compassionate physician whose primary goal is to maximize her patients’ functional ability and improve their lifestyle.  She prides herself on helping patients recover and reach their wellness goals.

She earned her Bachelor of Science degree at NYIT and her Doctor of Osteopathic Medicine degree at New York College Of Osteopathic Medicine. She completed residency in Physical Medicine and Rehabilitation at Stony Brook University Hospital, where she served as chief resident.  She began her practice at St. Charles Acute Rehabilitation and the Northport VA, where she treated patients suffering from stroke, traumatic brain injury, spinal cord injury and limb loss.  Her areas of expertise include limb loss rehabilitation, spasticity management, electromyography and sports medicine.

Genevieve McGerald, DO, CWS – P

Dr. McGerald is a dedicated wound care physician committed to improving the lives of her patients through comprehensive wound management and healing strategies. Utilizing the latest advancements in wound care technology and evidence-based practices, Dr. McGerald treats patients in both the office setting as well as in various rehabilitation and wound care centers on Long Island.   She attended medical school at the University of New England College of Osteopathic Medicine, graduating in 2005. 

She then completed a residency program for emergency medicine at Good Samaritan Hospital in West Islip, NY in 2009.   While working full-time as an emergency medicine physician, Dr. McGerald completed her wound care and hyperbaric medicine certification in 2018.  During this intensive training, she gained invaluable expertise in advanced wound assessment techniques, wound debridement, infection management, and the application of innovative wound care modalities such as negative pressure wound therapy, bioengineered skin substitutes, and hyperbaric oxygen therapy. 

She takes the time to thoroughly evaluate each patient’s unique medical history, lifestyle factors, and wound characteristics to develop personalized treatment plans tailored to optimize healing outcomes.

Nicole Grehn, AGPCNP-C

Nicole found her path to physiatry through her own experience as a patient. She became a bilateral above knee amputee in 2015 at the age of 24 due to complications after a rare heart condition that almost took her life. She graduated from Marquette University with a Master’s Degree in Nursing while she was learning to walk again, and decided to further her education to become a nurse practitioner so she could make an even greater impact in the disabled community.

Since that time, she has dedicated her days to helping people all over the world regain their independence after limb loss. In her current role at PMRLI, she is more motivated than ever to improve the lives of every patient she meets that is struggling to find their mobility. When Nicole isn’t working, she loves to travel and spend time with friends and family. 

Oswaldo Garcia, FNP

Oswaldo Garcia a board-certified nurse practitioner. He practiced as a registered nurse for six years before pursuing an advanced degree in family health. He has found his interests in prevention, lifestyle changes, and functional mobility.  Oswaldo has experience in neurosurgical and orthopedic care, as well as extensive training in diet and weight-loss strategies.  He is dedicated to the improvement and well-being of all his patients. He has developed an interest in amputee care, managing complex pain syndromes, and helping his patients achieve greater independence in functional mobility. 

Gary Hanson, PT

Gary Hanson obtained his physical therapy degree from Stony Brook University, NY in 1993.  Prior to this, he worked as a physical therapy assistant for eight years. Gary has experience in every area of physical therapy, including inpatient, outpatient, homecare, and rehabilitation hospitals. Gary is neurodevelopmental therapy (NDT) trained and has completed specialty training in traumatic brain injury and stroke rehabilitation. 

Additionally, he has extensive experience in the care of amputees, orthopedic injuries, and neurologic impairments. Gary readily adapts treatment plans to his patient’s needs, ages, precautions, and goals.  Gary leads an active lifestyle which includes regular participation in ice hockey, running races, various outdoor activities, and he maintains a consistent gym regimen. His goal is to remain healthy through correct exercise, diet, education, and outlook.  He aims to inspire his patients to achieve their best physical selves, and in that journey, find a deeper self-appreciation. Gary gains added strength in seeing others improve and overcome. We are all in this together.

Tara McGrath, PTA, MBA

Tara McGrath is a native to Long Island and is proud to be serving the Brightwaters and Patchogue communities. She is board certified and licensed in the state of New York and New Jersey as a physical therapy assistant since 2015 from Suffolk County Community College. She went on to receive her Bachelor’s Degree in Health Sciences from Southern New Hampshire University in 2018, and her Master’s in Business Administration in 2023. 

Tara is committed to helping patients return to living life to the fullest. She feels passionate about helping every patient reach their own personal goals. She has extensive experience and training in sports medicine and orthopedic injuries, neurological and cardiovascular disorders, geriatric and pediatric care, concussions, and amputee care. 

When working with patients, her goal is to develop strong working relationships with her patient’s healthcare team to achieve the best functional outcomes.  She addresses cardiovascular endurance, strength, and mobility training to help patients recover from their injuries and prevent further illness. Tara feels so grateful to all her patients for their trust as she walks them on their journey to a better, more fulfilling life.

Alexandria Reneiris, OT

Alex graduated from The State University of New York at Buffalo with her Master’s Degree in Occupational Therapy (OT) in June 2022. Raised on Long Island, Alex returned home to help the patients in her community regain their independence after injury or illness.
She began her career at Physical Medicine and Rehabilitation of Long Island in December 2022. 

She is passionate about providing individualized, patient-centered care utilizing her education, creativity, and problem-solving skills.  Her goal is to empower her patients with the education and tools they need to succeed and to live their most independent and meaningful lives.

Alexandra Lecuit, OTR/L

Alley received her Bachelor’s Degree in Psychology from SUNY Oneonta in 2016 and went on to receive her Master’s Degree in Occupational Therapy (OT) from Stony Brook University in May 2021. From a young age, Alley knew she wanted to make a difference in people’s lives and found her passion through occupational therapy, where she works with individuals of all ages to regain independence to participate in meaningful occupations.

She has experience working with orthopedic, neurological, sensory processing disorders, developmental delays, and post-surgical diagnoses. In her free time, Alley enjoys spending time with her friends and family and participating in activities including horseback riding.

Diane Arrighi– Medical Assistant

Diane has been with Physical Medicine and Rehabilitation of Long Island for more than six years.  She graduated from Bernard Baruch College (CUNY) with a Bachelor’s Degree in Business Administration.  She has multiple certifications in the field of Medical Assisting.  Her goal is to ensure that every patient feels confident and comfortable that our clinicians are providing the best care possible for their medical concerns.
